Enhancing Shoe Tying Skills With The Shoe Tying Club

Due to the frustrations experienced by their children, many parents tend to choose the easy way out buying shoes that do not require tying. Fast forward a year or two, parents find themselves frantically tying their second grader’s shoes on the sidelines of a soccer game or just before the bus. These parents are silently berating themselves for having given in earlier and are hoping for a miracle. First and second grade teachers are similarly frustrated when children arrive in their classroom lacking a skill that should have been learned in kindergarten. Deb Vozel, an intervention specialist at Cline Elementary School in Centerville, OH decided she needed to do something about it. Together with Bhanu Raghavan, OTR/L she started a shoe tying club to coach any second grader lacking shoe tying skills. The club turned out to be a resounding success!

Shoe tying club met during the second quarter of school. The club members were chosen by their classroom teachers, and included both typical and children with special needs. The steps used for shoe-tying came from the poster One-Two Tie Your Shoe. This poster was adapted from the book Self-Care with Flair! The club members were placed in groups of four to five. They met daily before the end of their morning session. Mrs. Vozel and her aide led the club daily with weekly consultation from the OT. Club members practiced a few steps each week. Once a step was mastered they were introduced to the subsequent step. If a child had a particular difficulty with mastering a step the OT helped to break the steps down even further (activity analysis). At the end of the second quarter all the club members were successfully tying their shoes. Mrs. Vozel celebrated their success by giving each member a certificate and a copy of the poster One-Two Tie Your Shoe.

The nine easy steps in One-Two Tie Your Shoe are a convenient and quick way to teach this skill to any child. The laminated poster can be displayed in the classroom, on the refrigerator at home, in day-care centers and wherever young children are learning to tie their shoes.

Developing Finger Dexterity

After babies discover how much fun it is to shake a rattle that fits comfortably inside the palm (at around 6 months of age), they gradually develop coordination between the index and middle fingers and thumb to grasp small blocks and eventually tiny cereal pieces. They also learn to use the index finger to point and push buttons. This is the beginning of what is called finger isolation – moving fingers separately. Refining finger isolation and the pincer grasp will enable the 2-3 year old child to control beads while stringing.

Some older children find it difficult to grasp tiny objects. They may use a “raking” motion to gather pennies inside the palm because they don’t have the coordination to pick them up using their fingertips. Difficulties may be related to decreased:

  • Attention and experience engaging in fine-motor activities
  • Hand strength and muscle tone
  • Sensory awareness
  • Abilities to plan and perform movements

Cause and effect toys such as Poppin’ Pals (by PlaySkool) and other “busy” toys with buttons have been around for a long time. In fact, this photo of my son exploring what his fingers can do was taken 25 years ago!

Pushing the buttons develops finger strength and motor control. These days, many toddlers learn to isolate their index finger by pressing icons on a smartphone or tablet. These devices do not develop finger strength and tactile learning. However, using the index finger to press, swipe and drag may be just what is needed to engage children with attention challenges. These are the kiddos who may prefer running around than stacking blocks. Controlling exciting visuals and sounds on a screen may initiate an interest in hand activities.

Developing Coordination Between Tripod Fingers

Children with autism, Down syndrome and other developmental disabilities often have decreased hand strength, muscle tone and sensory awareness. They benefit from toys that involve squeezing, pushing and pulling because these motions provide sensory input to the child’s muscles, joints and skin – while the child is having fun.

One very simple activity is to push objects through a small container opening. Start out with easy to grasp objects such as super balls or blocks. Next, introduce large poker chips, and then smaller Bingo chips. At first, be sure to make the opening large enough to ensure success, and then smaller so that the child uses force to push the objects through.

Occupational therapists love to see children push coins into piggy banks or any container with a slot because this requires coordination between the same fingers used to control a pencil – the middle and index fingers and thumb (sometimes called the tripod fingers).

Adapting for Success

Instead of using string, this toddler is grasping thick cord that is easier to control. I cut the donut shapes from container lids and plastic bottles. The holes can be cut larger or smaller depending on the child’s abilities. This adaptation makes it easier for typically developing children to learn how to sequence the steps to stringing, but it is especially exciting to tell parents of a child with a coordination challenges that their child is able to string. Children who do not yet have the motor control to manipulate beads may benefit from the adapting stringing activity shown in the photo.

Developing Finger Dexterity

I have noticed that some children grasp objects loosely, using their fingertips with their wrist flexed (bent). Encourage these children to play with toys that require force – such as turning the key to find a toy, pulling apart pieces of Velcro, and opening snaps – to strengthen the “tripod fingers” and develop dexterity.

Many years ago I designed the simple weaving shape shown in this photo.

The rectangular shape is inserted in and out of the large notches in the shape shown on the right. This requires manipulating with the fingertips. The beauty of making this activity (out of detergent bottles) is that I could cut them to any size. I have used these in color matching and sorting tasks with children and adults of all ages.

Weavable Toys

I have recently designed the “weaving belt” shown below. The small shapes woven onto a long plastic strip cut from a large round bottle (i.e. a bleach bottle) are perfect for small hands. Young children can weave them into color and/or shape pattern.

My clients have enjoyed weaving shapes so much, I decided to start manufacturing my patent-pending Weavable Toys: Basic Shapes.

These consist of

  1. 4 shape boards
  2. Strips to weave through the boards
  3. Shapes that are woven onto the strips to create designs.

The little girl in the photo is attaching shapes onto a circular strip without using a board. She does not yet have the skill to weave the strip into the board. This is a great example of a toy that can be adapted for many different ability levels.

Children can:

  • Put shapes on or off the strips without using the boards
  • Weave or remove strips from the boards without using the shapes
  • Weave shapes onto strips and into the boards to create designs (see photo)
  • Follow visual patterns to develop visual perceptual skills.

Whether purchasing or making your own, stringing, lacing and weaving boards are wonderful activities that develop the finger dexterity children need to control a pencil.

Using Movement to Motivate and Develop Hand Skills

Child on a horse as an example of using movement-based activities to develop hand skillsMovement, whether on a horse, swing or trampoline, provides sensory stimulation that nourishes the brain and in turn promotes attention and learning. Movement can also be used therapeutically to motivate children to engage in hand activities.

As an Occupational Therapist working at Ironstone Therapy in Andover, MA, I am able to use a horse as a treatment tool to work on traditional objectives such as improving bilateral coordination.

Hippotherapy is often very effective because children love bonding with the horse. Instead of in an office, this kind of therapy takes place in a happy environment where simply sitting on top of the animal is the first of many small successes.

Let’s begin by looking at Amber, a hippotherapy client. When I first met two-year-old Amber she arrived singing “Old McDonald Had a Farm.” She continued to sing while mounting and riding. She never looked at me, and seemed unaware of what her body was doing, and also unaware of the people around her.

Fortunately, Amber loved movement. She didn’t like it when I stopped the horse. I began singing “If You’re Happy and You Know It…Clap Your Hands/Pat the Pony/Blow a Kiss,” etc. At first, I used hand-over-hand assistance to help Amber perform the hand movements. When she ignored me, I stopped the movement. Amber soon learned that by engaging and using her hands, she could avoid stopping.

This example describes how movement can be used as reinforcement. A “reinforcer” is anything that makes a person want to repeat a behavior. Examples of reinforcements can be hugs, stickers, special treats, and even paychecks that motivate one to keep going to work.

Movement can function as a “sensory reinforcement” when it:

  • Motivates the child to repeat the desired behaviors (i.e. clapping hands)
  • Provides sensory input that organizes the brain

An important aspect of developing hand skills is learning to tolerate touch. Children receive deep pressure stimulation to joints and muscles as the horse gently bounces up and down during the walk.

Children may assume different positions, such as facing backwards while bearing weight on forearms or hands. Bearing weight helps to decrease touch sensitivities (i.e. tactile defensiveness) so that the child is more willing to grasp and manipulate objects.

Occupational and physical therapists often position children in “prone” position (on the belly) using swings, therapy balls or a scooter board so that children:

  • Develop postural control
  • Strengthen trunk, shoulders, neck and eye muscles
  • Bear weight on hands to decrease tactile defensiveness
  • Receive vestibular (movement that stimulates the inner ears) sensory stimulation that organizes the brain
  • And of course, are motivated – because movement is fun!

Scooter boards can also be used to develop sequencing and bilateral coordination as children “scoot” to a desired activity. The child might also be asked to scoot back and forth to bring game or puzzle pieces to a board. The clever mom at Fumbling Thru Autism motivates her daughter by integrating movement activities throughout the day.

Children can engage in a variety of hand activities while on a horse or other movement apparatus to develop basic skills such as reaching and grasping and more advanced skills such as:

  • Stabilizing with one hand while manipulating with the other
  • Orienting shapes to fit through openings (developing visual perceptual skills)
  • Sequencing several steps

For example, the client shown in this photo is inserting Lotto cards into the slots I cut into a detergent bottle. The slots are cut in vertical, horizontal and diagonal orientations so that she can practice positioning the cards to fit. Some of my more experienced clients can use this activity while kneeling or standing on top of the horse!

This child is using a magnetic Bingo wand to catch the goldfish while developing visual attention, postural control and sequencing skills as he removes the fish to place in an attached basket. The fish is cut out of a detergent bottle and has a paper clip attached so that it can be used in magnet games. This activity also helps children to coordinate using their hands together as they remove the fish from the Bingo wand.

Most therapists and parents do not have a horse available to use for sensory stimulation and reinforcement. However, the principles are the same when using other movement equipment – or no equipment at all. Young children may be twirled around after putting rings on a stack and older children may enjoy performing jumping jacks as they spell words. Most children of any age or ability level love to move and are motivated to communicate “go” with gesture, pictures or vocalizations. So what are you waiting for?
